The Form-A-Lines design studio is located in St. Albans, Hertfordshire, England.

The designer for Form-A-Lines is David Jefferson.

David originally created Form-A-Lines stitching card patterns for the Card Inspirations card craft supplies shop in Tewin when it was owned by his wife Linda and her business partner Anne. The first Form-A-Lines patterns were launched in 1998. Since then David has designed hundreds of patterns as D J Designs.

When the Card Inspirations business changed hands in August 2007 David and Linda acquired the rights to the patterns and the Form-A-Lines registered trademark. They left Card Inspirations to set up this business to wholesale Form-A-Lines patterns to retail craft shops.

David also has a totally separate business called Stitching Cards. This was started in June 2004 to market a different range of card stitching designs for direct download to your computer.

David has a background in graphic design and was the editor of Animator magazine between 1982 and 1995.
